Fourth Annual NC Conference—November, 2005
The Fourth Annual North Carolina Conference for Survivors of Suicide will be held at the UNC-Chapel Hill School of Social Work on Saturday, November 19, 2005, 8:00am-4:00pm. A brochure will be available online soon, but in the meantime, biographical information about our keynote speaker and registration information are available below.

Registration
The following information is required for registration:
- Primary attendee name
- Address of primary attendee
- Phone number of primary attendee
- Email address of primary attendee
- Names of any additional attendees (leave room for like 4)
- Relationship of each attendee to the deceased
- Lunch sandwich preference for each attendee: meat, turkey or vegetable

$15 per person, $10 for each additional family member
